What is Cognitive Attention Training?
                                                                              (Brain Training)


    People who have suffered brain damage from accidents and disease have been successfully rehabilitated for
    many years through cognitive rehabilitation therapy.  The same kinds of procedures, when applied to people
    with developmental disorders such as ADHD and learning disorders, are called cognitive training or brain
    training.

    Cognitive training consists of a variety of exercises designed to help improve functioning in areas such as
    sustaining attention, working memory, listening, visual and auditory information processing - areas in which
    ADHD and learning disabled individuals often experience difficulties. The principle underlying cognitive brain
    training is to help improve the core mental abilities that regulate information processing ability and learning
    and are necessary for successful academic performance and learning.

    Research has found that it is possible to improve attention and learning ability through systematic cognitive
    training (remediation).  Successful training is like physical exercise and requires frequent mental workouts to
    successfully strengthen cognitive functioning.  Cognitive training has improved attention, memory and
    learning problems resulting from many disorders such ADHD, ADD, learning disorders, autism, brain damage,
    consequences of aging and stroke.

    The use of medication is the primary and most effective treatment for ADHD.  However, for some children
    medication by itself is often only partially effective and functioning can be enhanced by combining medication
    and cognitive training.   Sometimes medication has unacceptable negative side effects and cannot be used.
    Medication is sometimes rejected because of reservations about the use of medication and alternative
    treatments are necessary.   

    Tutoring and practice in academic areas are often not effective. The principle underlying cognitive brain
    training is to help improve the core mental abilities that regulate information  processing ability and learning
    and are necessary for successful academic performance and learning.  


                                                        Can't Come To The Office Four Times a Week?

    Benefit from this type of training requires a minimum 20 hours training consisting of 4 days a week of at least
    half-hour daily sessions.  Four office visits a week would be much too expensive and impractical for most
    families.


                                                                                       The Solution

    The most practical, cost efficient way to provide this training is to use your own home based computer and
    special software that administers the remedial exercises, which are presented in computer game format.   
    This software allows me to tailor the exercises to fit the child (or adult), monitor progress and modify the
    exercises to match cognitive functioning as it changes.  This will allow us to provide intensive training at
    home, with only periodic office consultations for monitoring and software adjustment.   


                                                                      Can't I Just Buy A Program Myself?

    Yes, you can.  However, don't just by any system offered on the internet.  Just any computer like game will
    not work. I can provide the correct software at a substantial discount over what you would pay if purchased
    on your own and provide the consultation services needed to get the most out of  the system. It is important
    to use a system that is designed using scientifically established principles of cognitive remediation.  Such
    systems are complex and offer a wide variety of exercises intended to address dysfunction in different
    components of the cognitive system.   Each person’s pattern of cognitive strengths and weaknesses are
    different.  

    Consequently, supervision of  cognitive attention treatment by a Neuropsychologist is recommended.    



                                                                            The Treatment Program

    I have reviewed many different systems available and use the Captain’s Log because it is comprehensive
    and not limited to one of the component abilities required for learning.  The flexibility of the Captain's Log
    system makes it possible to keep the charge as low as possible.

     The
Captain’s Log system is the most comprehensive cognitive brain training system ever developed and
      includes 35 multi-level programs and provides more than 1000 hours of game-like brain training for ages 5 to
     105.
   Children benefit from the Captain's Log system and so do Adults.You will never need any other
     cognitive brain training software!

      Captain's Log can be configured for children or adults to develop, improve and remediate attention,
      concentration, memory, hand-eye coordination, fundamental numerical concepts and problem
      solving/reasoning skills.


      The initial set up for using the program consists of 2 appointments (Neurobehavioral Status Exam-CPT
       procedural code 96116) to measure and develop an individualized program for each patient.  Total Cost for 2
       initial appointments and software is $700.  A Captain’s Log CD (Captain’s Log @ Home)  will be provided
       with a software license lease that lasts one year.

                                                                        Insurance Reimbursement

    A diagnosis is not necessary to receive treatment through the Captain's Log system.  However, this treatment is
    not eligible for insurance reimbursement without the inclusion of an established diagnosis on the billing
    statements.

    The cost of the Captain's Log software is not usually eligible for insurance reimbursement, though your insurance
    may provide for such reimbursement and you are urged to check with your insurance.

    The appointments used to evaluate (96116) your functioning and used to tailor the Captain's Log software for your
    needs may be reimbursable from your insurance company.  However, insurance companies require that a diagnostic
    code be included in the billing statement in addition to the procedure code.  If you have been evaluated
    previously by Dr. Smith, then the diagnosis has been established. A new evaluation will need to be conducted If
    you have not been evaluated by anyone previously. Check with your insurance company.

    An evaluation and diagnosis by another provider is sufficient to document your diagnosis. A new evaluation is not
    necessary  All you will need to do is provide Dr. Smith with a copy of the evaluation report or brief letter from
    that provider that documents your diagnosis.  This may involve a copy of a neuropsychological evaluation,
    psychological evaluation or a brief note or letter from your physician or psychologist that includes that
    diagnostic code form the DSM-IV or ICD-9 diagnostic manuals.


                     
    It is recommended that formal consultations (CPT procedural code – 96116) should be done after 20, 40, 60
    and 80 hours of cognitive training to assess improvements and modify the training program for each person.  
    Cost - $150 per consultation.
