Dr. Smith specializes in the testing and diagnosis of Dyslexia, Attention Deficit Disorder, ADHD, ADD,  Independent
Educational Evaluation, Learning Disabilities and evaluations for the  Michigan Dyslexia Institute.

Evaluations can be scheduled through Dr. Smith's Okemos, Michigan office or through the Michigan Dyslexia Institute
offices in Lansing or Berkley, Michigan.

People seek evaluations for a variety of reasons.  

Frequently, clients seek a "second opinion" evaluations after a first evaluation by another psychologist is inclusive
regarding learning disabilities. These second opinion evaluations are frequently completed as an Independent Educational
Evaluation, which paid for by the school system as required by Special Education laws.  Clients seeking to qualify for
an Independent Educational Evaluation should consult the Special Education departments of their school systems.  
Individual school systems vary in how much they will pay toward an Independent Educational Evaluation.  Some
Special Education departments will pay for a large portion of the fee with the client or parent paying the remainder of
the fee.  

One of the most common reasons for an evaluation are to qualify for special accommodation under the Americans with
Disabilities Act  ( ADA ).  Requests for such accommodations must demonstrate through the application material, the
written report of the results of tests and any supportive materials from the schools Special Education department that
the learning disabilities or attention deficit disorder causes a substantial interference in educational activities or work
related activities.

THE WRITTEN REPORTS PREPARED BY DR. SMITH FOLLOW THE EVALUATION AND REPORT
GUIDELINES SPECIFIED BY THE ASSOCIATION ON HIGHER EDUCATION AND DISABILITY (AHEAD).  
WRITTEN FOLLOWING THE AHEAD GUIDELINES ARE RECOMMENDED WHEN APPLYING FOR
ACCOMMODATIONS UNDER the Americans with Disabilities Act  Amendment( ADAA).


The following list of prices are estimates for your convenience to give clients a realistic idea of the price.  The prices
stated are based on the battery of tests and procedures required for a typical evaluation for the stated condition.  The
procedures needed for a specific client is deterimined during the intake interview.  A specific client’s situation may
dictate that the price may be more or less depending on the number of hours (units) required to complete the evaluation.


Psychiatric Diagnostic Interview: (cost $190) (procedure code 90801).  
A psychiatric diagnostic interview is conducted to review the patient’s history, mental status, and symptoms, as well as
any previous testing.  In addition to dyslexia, other learning disabilities, attention deficit disorder and other psychological
conditions are considered during an evaluation.  Recommendations for further testing and/or interventions based on the
results of the interview are made and passed on to the client and any referring physician.   Dr. Smith makes it a practice
to recommend only the minimum evaluation procedures necessary to address each client’s concerns.  Often, a

Neuropsychological Evaluation for Developmental Dyslexia : (costs $1200) procedure codes 96101 (formerly 96100)  
or 96118 (formerly 96117) This is for the purpose of determining whether a child or adult has the reading problem that
meets the criteria to receive a formal diagnosis of a Reading Disorder (developmental dyslexia ).  It involves an
assessment of overall cognitive functioning as well as reading and reading related tests.  A formal diagnosis is necessary
if you are seeking insurance reimbursement or are trying to obtain special accommodations in school, for standardized

Neuropsychological Evaluation for ADHD : (cost $1200) procedure codes 96 118 (formerly 96117) or 96101 (formerly
96100).   This is for the purpose of determining whether a child or adult meets the criteria to receive a formal diagnosis
of  ADHD or ADD.  It involves an assessment of attention and related cognitive capacities through questionnaires,
rating scales and selected tests.  This test results in an integrated written report.

Record Review: (Cost $0-$315 depending on size of record) (Procedure  Code 90885)   Evaluation of hospital records,
school records, previous psychological reports, previous tests, and other accumulated data.  This results in a written
summary of conclusions and recommendations based on the review.   There is no charge if the Record Review is
conducted in the process of completing the Neurobehavioral Evaluation or one of the Neuropsychological Evaluations
and the results of the review are incorporated into the written report for those procedures.  A Record Review
conducted during the process of a Psychiatric Diagnostic Interview may require a separate fee for the Record Review
depending on the size of the record and the additional time required.  However, Record Reviews often do not require
significant additional time and in such cases additional fees will not be charged.   Click on Record Review


Neuropsychological Evaluation for Other Conditions: (cost based on procedures determined that diagnostic interview or
recommended from prior test results)  procedure code 96 118 (formerly 9117)    Occasionally  additional testing of
functions such as memory, executive functioning and motor functioning are warranted.


Explanation of Results-Interview: ($150) (procedure code 90887)   This is a one-hour appointment that can be
scheduled if additional explanation is desired in addition to the written report.


PSYCHOTHERAPY AND COUNSELING (Cost $110 per hour) Procedure Code 90806.: Most problem areas are dealt
with including depression, anxiety, self esteem and problems arising from a learning disorder.
